New Patient Information
We always welcome new patients! We will be in operation Monday through Thursday from 8am to 5pm, and on Fridays from 8 am to 12 pm. If you have a toothache we will make every effort to see you today. The vast majority of new patients call for a dental check up and cleaning, and our dental hygiene staff are ready to help. We keep blocks of time available for our new patients and can nearly always schedule a cleaning within 2 or 3 weeks, with emergency times available much sooner.
Regardless of your need – emergency or routine care – we are humbled to be selected as your dentist. It is a responsibility we take seriously. THANK YOU!

For your convenience, you can fill out forms securely online or print all of our New Patient Forms in PDF format prior to your first visit. Please fill them out and bring them with you!
PLEASE FILL OUT EACH FORM – Medical History 2 is for patient aged 18 and up.
Rhoades Family Dentistry files all insurance claims for patients as a helpful courtesy. We can do a complimentary benefits check if you provide your insurance information prior to your visit.
We process and e-file insurance for ALL insurance companies that give the patient their choice of Doctors. Even if we are not contracted (or “in-network”) providers through your carrier, if you are given your choice of provider we can assist you in understanding your plan. The practice has been a Premier Delta Dental Provider for over 20 years.
We always urge our patients to check with their insurance carrier or employer to understand their policy and its benefits. We do our best to estimate benefits for our patients but recommend they check with their carrier or employer.
We accept MC, Visa, American Express, Discover, Care Credit, debit cards, personal checks, and of course, cash!
We also offer Care Credit as a 3rd party financing option.
